How to Read Your 3D or 4D Ultrasound Picture
Everyone nods at the screen and then stares at the printout at home wondering which end is which. Here is how to make sense of it.

The Short Answer
A 3D ultrasound shows the surface of your baby as if lit from one side, so raised areas (nose, lips, cheeks, hands) look bright and recesses (eye sockets, the gap between lips) look dark. Find the nose first — it is usually the brightest small bump near the middle — then the lips below it and the closed eyes above. Anything cutting across the face in a smooth arc is almost always the umbilical cord or an arm.
Flat, squashed or missing areas are not features of your baby: they are places where the placenta, the uterine wall or a hand was pressed against the face, so no fluid gap existed for the sound to read. This page is general orientation, not medical interpretation; your sonographer is the person to ask about anything on the scan.
How to Orient the Picture
Find the nose
The brightest small bump near the centre. The face is arranged around it.
Trace down to the lips
Two soft ridges below the nose with a darker line between them. The chin is below that.
Look for what is covering the face
A smooth arc across the cheek is the cord; a splayed bright shape is a hand. Neither is part of the face.
What Bright and Dark Actually Mean
The software builds a surface from reflected sound and then lights it from one direction, like a studio lamp. Surfaces facing that light render bright; surfaces angled away or set back render dark. So a bright patch is not “dense” and a dark patch is not “missing” — they are simply facing towards or away from the virtual light.
Colour is added by the machine. Sepia, gold or pink tints are presentation choices; the scan has no colour information about your baby at all.
Common Things That Confuse Parents
“The face looks flat on one side”
Something was pressed against it: placenta, uterine wall or a hand. The face is fine; the sound could not get behind the obstruction.
“There is a hole in the cheek”
A shadow cast by the cord or a hand, or a gap where fluid was missing. Not a real hollow.
“The eyes look sunken”
Eye sockets are recesses and render dark. Closed eyelids read as smooth shapes inside them.
“The face looks bony”
Common before about 26 weeks, when fat under the skin has not yet filled out the cheeks.

Can I tell what my baby looks like from the scan?+
